Registration Forms

Registration for the HCYA organization is now ONLINE. If you do not have a SportsEngine account, you will need to create one and pay the $20 registration fee. Please wait until after tryouts.
 

All families need to print out these forms and turn them in at the parents meeting:

• HCYA Medical Release Form
• Code of Conduct Form

All coaches and volunteers need to turn in this form:

• Volunteer Background Check Form
